Application Form for Permissions

For your convenience, a standard form for applying for journal permissions is provided. Please complete the form and fax, email or mail it to the Publications Department to the address below.

The Society of Nuclear Medicine handles all requests to reproduce material from SNM journals or books.

The department is staffed from 8:00am-5:00pm, from Monday to Friday. To contact the department at any other time, please send an email message or fax.

The current turnaround time for processing permission requests is 7-14 days of receipt. All responses are sent by mail, unless an immediate deadline is clearly indicated on the request, and a fax number is provided.

If faxing or mailing a request, please ensure that you include the full reference of the material you want to use (journal name, volume and issue numbers, author, title of article, figure numbers where appropriate, etc). We also require a detailed overview of what you would like to use the material for (i.e. in a book, etc.)
